ABOUT THE HEALTH OFFICE: We are on duty during school hours (7:10-2:20) and available to students who become ill or injured during school hours. The health office is also available to parents with questions regarding their children and/or school health policies. We welcome and encourage calls to update us regarding your child’s health concerns and needs.

SCHOOL ENTRY INTO 5th GRADE: Upon entry into 5th grade and in accordance with Dover School Board Policy JLCA, we require a current physical exam report (within the past year from the first day of school) to be submitted and that immunizations are up to date. Physical Exam Form

SCHOOL ENTRY INTO 7TH GRADE: In accordance with NH State Immunization
Requirements, all students will require a TDAP vaccine in order to enter 7th grade. This vaccine is often given once a child reaches age 10. The school will require that documentation of that vaccine administration in order to enter 7th grade.
ILLNESS: To ensure a healthy environment we ask that students who have had diarrhea, vomiting, and/or a temperature over l00 degrees should be kept home until free from symptoms, without the aid of medication, for not less than 24 hours. A child with a bacterial infection that can be spread to other children (strep throat, impetigo, etc.) must be on an antibiotic for at least 24 hours before returning to school.
EMERGENCY PLANS
IF YOUR CHILD HAS ASTHMA , AN ANAPHYLACTIC ALLERGY OR SEIZURES
Your child needs an EMERGENCY ACTION PLAN at school. These emergency plans are the tools needed to keep your child healthy. Each school year a new plan is required. The plan must be signed by both the parent/guardian and the Health Care Provider. Forms must be completed and brought into the health office (along with any required medications) before the first day of school. Links to blank Emergency Action Plans are provided below.
MEDICATION AT SCHOOL
Over the counter medications
If your child may require over the counter medication during school, the parent/guardian must sign a medication authorization form. Signed forms and medications can be brought into the health office. All over the counter medications must be dispensed from the health office and be in the original containers and labeled with your child’s name.
Prescription Medications
If your child needs prescription medication to be administered during school hours, please note the following:
Medication must be in a pharmacy labeled bottle with the student’s name & correct dosage, and contain only enough medication for one (1) month. Medication must be brought-in by a responsible adult. A written letter by your child's doctor which includes student's name, date of the order, the name and signature of prescriber and contact numbers, medication name, route and dosage of medication, diagnosis (if not a violation of confidentiality), directions/ frequency and time of administration, and possible side effects must accompany the prescription. We can accept this written letter from your provider, or the form linked below can be given to the provider to complete. Please ask the pharmacy to divide the medication into two bottles, one for home and one for school.
